This Consultation Services Agreement (the "Agreement") is made and entered into between Global Mobility Physiotherapy Centre (referred to as "Provider") and the client (referred to as "Client").

1- Services:

Provider agrees to provide consultation services to the Client through the website using the Zoom application. The consultations will be conducted by qualified physiotherapy consultants employed by the Provider.

2- Booking and Cancellation:

2.1 Client may book an appointment through the Provider's website.
2.2 Client has the right to cancel the appointment up to 5 hours before the scheduled session without incurring any charges.

3- Payment and Refund:

3.1 Client agrees to pay the agreed-upon fee for the consultation services at the time of booking.
3.2 If the Client cancels the appointment within the specified timeframe (5 hours prior to the scheduled session), the Provider will initiate a refund process.
3.3 Refunds will be processed using the original payment method, and the refunded amount will be returned to the Client within a reasonable timeframe.

4-Responsibilities of the Provider:

4.1 The Provider will ensure that the consultation services are conducted professionally and in a timely manner.
4.2 The Provider will make reasonable efforts to accommodate any necessary rescheduling requested by the Client.
4.3 The Provider will maintain the confidentiality of the Client's personal and health information in accordance with applicable laws and regulations.

5- Responsibilities of the Client:

5.1 The Client agrees to provide accurate and up-to-date information during the booking process.
5.2 The Client agrees to be available and prepared for the scheduled consultation session at the agreed-upon time.
5.3 The Client agrees to comply with any instructions or recommendations provided by the physiotherapy consultant during the consultation session.

6- Limitation of Liability:

6.1 The Provider shall not be liable for any direct, indirect, incidental, or consequential damages resulting from the consultation services or any actions taken or not taken based on the advice provided during the sessions.
6.2 The Provider shall not be liable for any technical difficulties or interruptions that may occur during the Zoom session, beyond the Provider's reasonable control.

7- Termination:

Either party may terminate this Agreement by providing written notice to the other party. Termination does not affect any rights or obligations that have already accrued under the Agreement.

8- Governing Law and Jurisdiction:

This Agreement shall be governed by and construed in accordance with the laws of [Jurisdiction]. Any disputes arising from or in connection with this Agreement shall be subject to the exclusive jurisdiction of the courts of [Jurisdiction].

By booking a consultation appointment, the Client acknowledges that they have read, understood, and agreed to the terms and conditions outlined in this Consultation Services Agreement.
